She's just finishing fourth grade, but her lowest score is almost sixth grade. "PHS" indicates she's performing at post-high school levels. Almost all of her checkmarks are in the "above average" category. There are a few that have an "H" next to them, which means that is as high as it can go in the "average" category before being bumped up to the "above average" zone.
